ABSTRACT

Objective:To investigate the value of three-dimensional (3D) CT in diagnosing cricoarytenoid dislocation.Methods:From January 2021 to December 2021, 31 patients with unilateral cricoarytenoid dislocation who had been treated by reduction forceps at the Affiliated BenQ Hospital of Nanjing Medical University were collected retrospectively, and their voice recovered or improved significantly after therapy. The preoperative CT images were reconstructed by volume rendering (VR). The dislocated side (left and right), type of dislocation (total dislocation and subluxation), and dislocation direction (anterior, posterior, internal and external dislocation) of cricoarytenoid dislocation were observed. According to arytenoid articular surface of cricoid cartilage exposed completely or not (caused by arytenoid displacement), they were divided into complete dislocation and subluxation. According to the direction of arytenoid displacement and the part of arytenoid articular surface of cricoid cartilage exposed, they were divided into anterior, posterior, internal and external dislocation. According to the shape of the vocal cords on laryngoscope, anterior and posterior dislocation of each case was judged, and then compared with that of CT.Results:On VR images, there were 28 cases of cricoarytenoid subluxation (90.3%, 28/31) and 3 cases of complete dislocation (9.7%, 3/31). Left cricoarytenoid dislocation was 26 cases (83.9%, 26/31) and right cricoarytenoid dislocation was 5 cases (16.1%, 5/31). Posterior dislocation was 28 cases (90.3%, 28/31) and anterior dislocation was 3 cases (9.7%, 3/31). There were 23 cases of internal dislocation (74.2%, 23/31), 2 cases of external dislocation (6.4%, 2/31), and 6 cases without obvious internal and external dislocation (19.4%, 6/31). Three cases of complete dislocation were left posterior internal dislocation.There were 24 cases of left posterior dislocation (77.4%, 24/31), 4 cases of right posterior dislocation (12.9%, 4/31), 2 cases of left anterior dislocation (6.4%, 2/31) and 1 case of right anterior dislocation (3.2%, 1/31). On laryngoscope, there were 19 cases of posterior dislocation (61.3%, 19/31), 9 cases of anterior dislocation (29.0%, 9/31), 3 cases were difficult to assess (9.7%, 3/31) because of aryepiglottic fold covering. Sixteen cases (55.2%, 16/28) were consistent with 3D CT, and 12 cases (42.8%, 12/28) were inconsistent.Conclusion:The 3D CT is a reliable method to evaluate cricoarytenoid dislocation, which can show dislocated side, type and direction of cricoarytenoid dislocation clearly.

ABSTRACT

ObjectiveTo investigate the mechanism of Zuogui Jiangtang Qinggan prescription (ZJQP) in improving glucose and lipid metabolism in loss of skeletalmuscle-specific insulin-like growth factor-1 receptor function (MKR) mice with type 2 diabetes mellitus (T2DM) and non-alcoholic fatty liver disease (NAFLD). MethodNAFLD was induced by high-fat diet feeding for 8 weeks in MKR mice, which were randomly divided into model group, metformin group (0.067 g·kg-1), and ZJQP high and low-dose groups(14.8, 7.4 g·kg-1). Ten FVB mice of the same age were used as the normal group. After 8 weeks of drug treatment, the oral glucose tolerance test (OGTT) was performed, the serum was taken to detect triacylglycerol (TG) and total cholesterol (TC), and the wet weight of the mouse liver was weighed. Haematoxylin-eosin (HE) staining and oil red O staining were performed to assess histopathology of liver. The mRNA expression and protein expression of Fork head box protein O1 (FoxO1), phosphoenolpyruvate carboxykinase (PEPCK), glucose-6-phosphatase (G6Pase), and apolipoprotein C3 (ApoC-Ⅲ) in liver tissues were detected by real-time fluorescent quantitative PCR (Real-time PCR) and Western blot, respectively. ResultAs compared with the normal group, the levels of fasting blood glucose, liver index, serum TG, TC, and OGTT of mice in the model group increased significantly (P<0.01). As compared with model group, the fasting blood glucose and liver index of the mice in the metformin group and the ZJQP group decreased significantly (P<0.01), the serum levels of TG and TC in the high-dose ZJQP group decreased significantly (P<0.05,P<0.01), and the OGTT of mice in the metformin group and the high-dose ZJQP group improved (P<0.05). In histopathology, as compared with the normal group, mice in the model group showed decreased lipid droplets and vacuoles in hepatocytes, and their volumes became larger. Compared with the model group, the ZJQP group and metformin group showed that the lipid droplets in liver tissues were reduced, the vacuoles in liver cells were reduced, and the volume was smaller. At the molecular level, as compared with the normal group, the mRNA and protein levels of FoxO1, PEPCK, G6Pase, and ApoC-Ⅲ in liver tissues of mice in the model group were significantly up-regulated (P<0.01). As compared with the model group, the mRNA and protein levels of FoxO1, PEPCK, G6Pase, and ApoC-Ⅲ in the ZJQP group was significantly decreased (P<0.01). ConclusionZJQP can improve the glucose and lipid metabolism of T2DM with NAFLD and repair the pathological damage of liver, which may be through regulating the expression of FoxO1, PEPCK, G6Pase, ApoC-Ⅲ-related proteins in liver tissues to achieve the effects of regulating lipid, lowering glucose, and delaying hepatic steatosis.

The human cytomegalovirus (HCMV) infection is widespread in the world, which mainly affects people with an insufficient immunity.Interferon is an important member involved in the innate immune, which is of great significance in the early antiviral response.Owing to the huge genome, HCMV expresses various proteins that resist the host immune response effectively.Moreover, HCMV also exploits host cells to resist the immune response of interferon, thus escaping from the host immune response and establishing a latent infection.This study aims to review the research progress of HCMV in escaping from the host interferon response.

ABSTRACT

Reporting statistical tests for baseline measures of clinical trials does not make sense since the statistical significance is dependent on sample size, as a large trial can find significance in the same difference that a small trial did not find to be statistically significant.We use 3 published trials using the same baseline measures to provide the relationship between trial sample size and p value.For trial 1 sequential organ failure assessment (SOFA)score, p=0.01, 10.4±3.4 vs. 9.6±3.2, difference=0.8; p=0.007 for vasopressors, 83.0% vs. 72.6%. Trial 2 has SOFA score 11±3 vs. 12±3, difference=1, p=0.42. Trial 3 has vasopressors 73% vs. 83%, p=0.21. Based on trial 2, supine group has a mean of 12 and an SD of 3 for SOFA score, while prone group has a mean of 11 and an SD of 3 for SOFA score. The pvalues are 0.29850, 0.09877, 0.01940, 0.00094, 0.00005, and <0.00001 when n (per arm) is 20, 50, 100, 200, 300 and 400, respectively.Based on trial 3 information, the vasopressors percentages are 73.0% in the supine group vs. 83.0% in the prone group. The pvalues are 0.4452, 0.2274, 0.0878, 0.0158, 0.0031, and 0.0006 when n (per arm) is 20, 50, 100, 200, 300 and 400, respectively.Small trials provide larger pvalues than big trials for the same baseline differences. We cannot define the imbalance in baseline measures only based on these pvalues. There is no statistical basis for advocating the baseline difference tests

ABSTRACT

Objective:To analyze the factors of missed diagnosis and misdiagnosis of gallbladder adenomyomatosis(GBA) by ultrasonography according to the contrast between ultrasonography and pathology.Methods:Four hundred and forty-six cases of patients with pathological diagnosis of GBA and complete preoperative case informations were retrospectively collected from January 2010 to November 2018 in the PLA General Hospital, and the preoperative ultrasound diagnosis, clinical symptoms, pathological classification were analyzed. The coincidence rate and missed diagnosis rate of GBA by ultrasound were calculated. The ultrasonic diagnosis of GBA missed diagnosis and misdiagnosis factors were analyzed according to the multiple factors and single factor Logistic regression.Results:The coincidence rate of ultrasonic diagnosis was 46.9%(200/426), the missed diagnosis rate was 35.2%(150/426), and the misdiagnosis rate was 17.8%(76/426). Misdiagnosed cases included 65 cases (85.5%) of gallbladder occupation, 7 cases (9.2%) of gallbladder stones, and 4 cases (5.3%) of chronic cholecystitis. The results of multi-fact OR Logistics regression analysis showed that gallbladder stones ( OR=5.112, P<0.001), chronic cholecystitis ( OR=4.613, P<0.001), gallbladder occupation ( OR=6.565, P<0.001) and cholesterol crystals ( OR=8.634, P<0.001) were independent influencing factors for accurate diagnosis of GBA by ultrasound. Conclusions:Ultrasound has a high rate of missed diagnosis and misdiagnosis for GBA operation group at present. Since most cases are complicated with other gallbladder diseases, it is important to scan carefully for GBA besides the diagnosis of a single gallbladder disease.

ABSTRACT

Objective@#To understand the changing characteristics of sleep quality of acute plateau nursing staff during the period of assistance to Tibet, formulate comprehensive intervention measures, improve the support ability of nursing staff, and provide the basis for effectively completing the work of plateau assistance to Tibet.@*Methods@#From November to December 2017, 12 nurses who participated in the assistance work for Tibet in Lhasa region with an altitude of 3,650 m were selected as the emergency group, and 12 nurses from a hospital in Tibet were selected as the plateau group. Pittsburgh sleep quality index (PSQI) was used to investigate the sleep quality of the acute group before and after Tibet entry and the plateau group respectively. A sleep quality survey scale was designed to investigate the sleep quality of nursing staff in the emergency group and the plateau group at 5 different shifts on the same night after admission to Tibet.@*Results@#The scores of total sleep score, sleep quality, sleep time, sleep time, sleep efficiency and daytime dysfunction in the acute group were 3.77±1.79, 0.89±0.47, 0.78±0.36, 0.76±0.41, 0.19±0.05, 0.54±0.07, and 5.37±1.23, 1.57±0.36, 1.53±0.43, 1.21±0.38, 0.72±0.10, 0.99±0.91 respectively. The differences before and after Tibet were statistically significant (t=2.327-4.399, P <0.05).The scores of sleep time, sleep time, sleep efficiency and sleep disorder in the acute group were 1.53±0.43, 1.21±0.38, 0.72±0.19, 1.16±0.21, and 1.04±0.21, 0.86±0.32, 0.39±0.31 and 0.76±0.33 respectively. The differences between the two groups were statistically significant (t=2.441-3.547, P<0.05 or 0.01).Radical groups after Tibet plateau and nursing staff sleep quality scale scores compare, radical group day shift, middle shift, night after night under three different shifts the total score of sleep quality, respectively 11.76±0.12, 11.98±0.23, 12.43±0.52, higher than that of plateau group 11.18±0.04 11.23±0.57, 11.98±0.54, the difference between two groups was statistically significant (t=15.554, 4.227, 2.07, P<0.01 or 0.05). The sleep quality score (11.38±0.36) in the night of the rest class was lower than that of the plateau group (11.92±0.38), and the difference was statistically significant (t=-3.574, P=0.002). The score of sleep quality and sleep delay of the acute group were 1.87±0.57, 1.93±0.61, and 1.39±0.39, 1.25±0.42 respectively. The difference between the two groups was statistically significant (t=2.408, 3.181, P< 0.05 or 0.01).@*Conclusions@#It is a common sleep problem for medical and nursing personnel in Tibet who rush into the plateau. Comprehensive intervention measures should be taken in advance, scientific popularization, education and health technical guidance should be carried out in the early stage, mental health conditions should be improved, and drug prevention should be taken when necessary to help medical and nursing personnel in Tibet sleep quality.

ABSTRACT

0bjective To evaluate the changes in blood oxygen saturation and pulse rate of the nurses under abrupt highland advancing and provide the fundamentals for those nurses under abrupt highland advancing to Tibet. Methods We selected 12 nursing staffs abruptly advanced to Lhasa at an altitude of 3 650 meters for Abrupt Advancing Group then chose 12 nursing staffs over there for Highland Group from November 2017 to December 2017. To assess whether there is a difference about the change of blood oxygen saturation and pulse rate between these two groups, we measured them in 24 nurses and 360 shifts before work and at 0.5, 1.0, 3.0, 6.0 h after work. Then, we compared the changes in blood oxygen saturation and pulse rate between different ages, sexes, shifts. Results No significant difference in blood oxygen saturation was observed between the two groups before starting work (P > 0.05). Blood oxygen saturation of Abrupt Advancing Group (0.5, 1.0, 3.0, 6.0 h) after work was respectively 0.91±0.01, 0.91±0.02, 0.91±0.02, 0.91±0.01. Meanwhile, the parameters of Highland Group was separately 0.94± 0.02, 0.93 ± 0.01, 0.94 ± 0.01, 0.94 ± 0.01. There was a significant difference between the two groups (t=5.153-8.151, P<0.05 or 0.01). Before work, the pulse rate was (85.38±2.38), (91.62±2.79),(89.79±2.05), (89.12±2.29), (89.72±2.31) times/min for Abruptly Advancing Group, (93.52±2.64), (93.52±2.82), (94.52± 2.73), (94.52±2.56), (93.52±2.96) times/min for Highland Group. The difference between the two groups was statistically significant (t=3.875-10.182, P<0.01). Besides, no difference about the changes in blood oxygen saturation of Abruptly Advancing Group between different shifts was observed (t=3.561-8.321, P<0.05 or 0.01). The difference of pulse rate in Abruptly Advancing Group was statistically significant (t=1.529-4.968, P<0.05 or 0.01). There was no significant difference of blood oxygen saturation and pulse rate in Abruptly Advancing Group between the age of 21 to 30 and 31 to 40 years old, whether before or after work (P > 0.05). A positive difference between the sexes could be found about the changes in the blood oxygen saturation of Abruptly Advancing Group at 0.5, 1.0, 3.0 and 6.0 h after work. What′s more, there was a significant difference in pulse rates before and after work (t=3.283-6.719, P<0.05 or 0.01). Conclusions The nursing staff in the Abruptly Advancing Group shows decreased blood oxygen saturation and increased pulse rate compared to those in the Highland Group during working period. In addition, the changes show more significant in those working at night, but they adapted gradually. Gender and working time have a significant effect on the change in blood oxygen saturation and increased pulse rate of the nursing staff.

ABSTRACT

Currently,medical imaging (ultrasound,mammography,CT,MRI) is of great value in diagnosis of breast lesions,and has a high sensitivity and accuracy in qualitative diagnosis.Based on medical imaging and with the rapid development,three dimensional (3D) printing and 3D reconstruction techniques can provide more information about accurate position,anatomical structures,morphology and volume of lesions and so on.And it is helpful in efficient communication with patient,making precise surgical plan,postoperative individual reconstruction and assessment of the effect after chemotherapy.The progresses of 3D reconstruction and printing techniques based on medical imaging in diagnosis of breast lesions were reviewed in this paper.

ABSTRACT

Objective To evaluate the effect of epidural labor analgesia with different concentra-tions of ropivacaine combined with sufentanil on fever. Methods A total of 104 healthy parturients, of A-merican Society of Anesthesiologists physical status Ⅰ or Ⅱ, with New York Heart Association gradeⅠ orⅡ, with body height 150-175 cm, weighing 50-90 kg, at 37-45 weeks of gestation, scheduled for elec-tive labor analgesia, were divided into 0. 075% ropivacaine group ( group R1, n=51) and 0. 125% ropiva-caine group ( group R2, n=53) by a random number table method. Epidural labor analgesia was performed with 0. 075% ropivacaine plus 0. 25μg∕ml sufentanil and with 0. 125% ropivacaine plus 0. 25μg∕ml sufen-tanil in group R1 and group R2, respectively, to maintain visual analog scale score<3. Body temperature was measured before analgesia ( T0 ) , at 30 min, 1, 3 and 5 h of analgesia ( T1-4 ) , immediately after de-livery and at 2 h after delivery ( T5,6 ) . Venous blood samples were collected at T0,3,5 to detect the concen-tration of interleukin-6 ( IL-6) in serum. The incidence of fever, plane of anesthesia, fluid infusion rate, consumption of sufentanil and ropivacaine, the number of increment of drugs and length of labor were recor-ded. Results Compared with group R1, the degree of increase in body temperature was significantly in-creased at T1-6 , the consumption of ropivacaine and concentration of IL-6 at T3 were increased in group R2 (P<0. 05). There was no significant difference in the rate of fever, plane of anesthesia, fluid infusion rate, consumption of sufentanil and ropivacaine, the number of increment of drugs or length of labor be-tween two groups ( P>0. 05) . Conclusion Epidural labor analgesia with different concentrations of ropiva-caine combined with sufentanil exerts no effect on fever, 0. 075% ropivacaine induces less changes in body temperature of parturients than 0. 125% ropivacaine, which is related to the lower concentration of IL-6.

ABSTRACT

Colon cancer is one of the common malignant tumors of digestive system.In our previous study, it has been demonstrated that colon cancer specific antigen thioredoxin like-2b (Txl-2b) can interact with Ras-related nuclear protein (Ran).However, there are few reports about the role and mechanism of Ran in tumorigenesis of colon cancer.Aims: To investigate the effect of Ran-targeting RNA interference on apoptosis and expressions of caspase-3 and poly(ADP-ribose) polymerase (PARP) in colon cancer cell lines.Methods: 20, 40 and 60 nmol/L siRNA-1 (si-1 group), siRNA-2 (si-2 group), siRNA-3 (si-3 group) targeting to Ran gene and normal control siRNA (NC group) were transfected into colon cancer cell line HCT116 and DLD-1, respectively.The interference efficiency of siRNAs and expressions of caspase-3 and PARP were detected by Western blotting.Cell apoptosis was determined by flow cytometry.Results: 20 nmol/L siRNA-1 and siRNA-2 had the best effect on inhibiting expression of Ran.Early apoptosis rates of HCT116 and DLD-1 cells in si-1 group were significantly higher than those in NC group (19.37%±7.57% vs.4.83%±1.72%;16.53%±3.38% vs.6.27%±3.13%;P all <0.05).Late apoptosis rates of HCT116 and DLD-1 cells in si-1 and si-2 groups were significantly higher than those in NC group (15.97%±3.31%, 16.33%±5.40% vs.6.40%±1.05%;22.93%±1.57%, 11.50%±0.70% vs.6.20%±0.98%;P all <0.05).Compared with NC group, expressions of cleaved caspase-3 (active form) and cleaved PARP (inactive form) were significantly increased in DLD-1 cells of si-1 and si-2 groups.Conclusions: Silencing Ran gene can significantly promote the apoptosis of colon cancer cells, and its mechanism is related to the regulation of caspase-3 and PARP expression.

ABSTRACT

Human cytomegalovirus (HCMV) infection is widespread. According to different economic conditions, the serology positive rate among people ranges from 40% to 100%. HCMV belongs to the β-herpesvirus subfamily, the virus contains an amazing enormous genome of approximately 250 kb that encodes over 200 and perhaps as many as 751 protein products. Notably, IE1 encoded by UL123 and IE2 encoded by UL122 are the most abundant and important among them.IE1 is a powerful transcription activator, which is also the first expressed protein after HCMV infection. Contrast to high MOI, IE1 is essential to virus replication when HCMV is in a low input.IE2 is important for inducing cytolysis effects, which also has an irreplaceable role for viral replication.IE proteins have involved in many mechanisms to resist the innate and adaptive immune responses to cytomegalovirus infection. What’s more, IE proteins are believed to have a role in regulating virus latency and reactivation state.

Objective To detect miR-181d expression levels in colon cancer cell lines,and to study the functions of miR-181d on colon cancer cell proliferation and apoptosis.Methods RT qPCR was employed to study miR-181d cxpression levels in colon cancer cell line HCT116,HT29,LoVo,SW480 and SW620 cells,as well as in colon normal epithelial cell line HIEC.miR-181d mimic and control were transfected into LoVo cells while miR-181d inhibitor and control were transfected into SW620 cells.qRT-PCR was performed to validate the transfection efficiency.MTT assay was performed to measure cell proliferation while flow cytometry was performed to detect cell cycle and apoptosis rate.Results miR-181d was universally downregulated in all colon cancer cell lines compared to the colon normal epithelial cell line HIEC (F=29.34,P<0.01).Overexpression of miR-181d in LoVo cells significantly decreased in vitro cell proliferation rate (F=5.403,P<0.01).Flow cytometry indicated that cells at S phase were greatly decreased (t=4.71,P<0.05) and apoptotic cells were gready increased compared to the control cells (t=3.47,P<0.05).On the contrary,inhibition of miR-181d in SW620 cells significantly promoted cell proliferation (F=20.82,P<0.01).Cell cycle was accelerated with significant increase in S phase compared to the control cells (t=2.92,P<0.05),whereas apoptosis rano was significantly decreased (t=4.14,P<0.05).Conclusion miR-181d was universally downregulated in colon cancer cell lines compared to the normal epithelial cell line.miR-181d inhibits cell proliferation and induces apoptosis,thus functions as an tumor suppressive miRNA.

ABSTRACT

Objective To compare the effects of propofol and ketamine on long-term memory and the expression of brain N-methylgroup-D-aspartate receptor 2B(NMDAR2B) and Gamma-aminobutyric acid receptor 1(GABAR1) in aged rats, and preliminary investigate the relation between the long-term memory and expression of neurotransmitter receptors in different cerebral areas. Methods The aged male rats were randomly divided into control group,propofol group and ketamine group. Morris water maze training was performed in all the rats of three groups for 5 days. On the 6th day, intraperitoneal injection of 50 mg?mL-1 propofol was administrated in propofol group,80 mg?mL-1 ketamine was intraperitoneally injected in ketamine group,and blank control group was given the same dose of saline.Seven days after the administration,space exploration experiment and navigation experiment test were performed to test the impact on the learning and memory ability of rats. After that, the expression levels of GABAR1 and NMDAR2B in temporal lobe and hippocampal CA1 region of the rat brain were detected by immunofluorescence and FISH technique. Results The results of Morris water maze showed there was no significant difference between propofol group (9.49±1.24) s and blank control group (8.82±2.22) s.There was statistically significant difference between ketamine group (12.04±2.67) s and blank control group (P<0.05),with longer latency time and less number of times of passing through target as compared with blank control group.By using immunohistochemistry and FISH technique,the expression of GABAR1 in temporal lobe and hippocampal CA1 region of the rat brain was not significantly different between propofol group and blank control group,but it was significantly up-regulated in ketamine group as compared with blank control group ( P<0.05) . The expression of NMDAR2B in temporal lobe and hippocampal CA1 region of the rat brain was not significantly different between propofol group and blank control group,but it was significantly down-regulated in ketamine group as compared with blank control group (P<0.05). Conclusion Propofol anesthesia alone had no effect on long-term learning and memory,but ketamine anesthesia can result in long-term learning and memory impairment. The mechanism may be related with down-regulation of the expression of NMDAR2B receptor and up-regulation of GABAR1 not only in CA1 region hippocampus,but also in temporal lobe.

ABSTRACT

OBJECTIVE@#To achieve targeted and meticulous surgery of adenoid hypertrophy, a comprehensive analysis of adenoid hypertrophy space occupying effect and morphological evaluation were conducted and the clinical results were retrospectively analyzed.@*METHOD@#One hundred and sixty-three children with adenoid hypertrophy were treated in our department from May 2013 to May 2014. All children received three examinations preoperatively, including: Nasopharyngo-fiberoscopy, Audiometry and Tympanometry. Based on the results, space occupying effect of adenoid hypertrophy was divided into three types: vertical hypertrophy type, horizontal hypertrophy type and vertical & horizontal hypertrophy type. We assumed the causal relationship with vertical hypertrophy type to snoring (nasal blockage) and horizontal hypertrophy type to secretory otitis media respectively. All children received transoral endoscopic adenoidectomy with radiofrequency ablation.@*RESULT@#The postoperative followup of these children for 6 to 12 months showed that the vertical hypertrophy type and horizontal hypertrophy type children all recovered from the syndromes of snoring (nasal blockage) and secretory otitis media respectively. The nasopharyngo-fiberoscopy showed that the nasopharyngeal space was smooth and the bilateral choanas opened well. No recurrence was found.@*CONCLUSION@#The philosophy of comprehensive analysis on adenoid hypertrophy space occupying effect could help the surgeons understand adenoid hypertrophy better and can guide the adenoidectomy more meticulously.

ABSTRACT

Objective To explore clinical and muscular pathological features of statin-induced myopathy.Methods Nine patients were enrolled in this study,who were diagnosed as statin-induced myopathy by muscle biopsy in Peking University First Hospital from April,2012 to October,2014.The clinical data and pathological findings were analyzed.Results The exposure time to statins varied from 4 days to 4 years in the total of 9 patients,6 males and 3 females,with the average age of 63 ± 6 (55 to 74) years old.Three patients suffered from myalgia and 6 patients complained of weakness mainly at the proximal limbs,while no symptoms occured in 3 patients.Serum creatine kinase (CK) increased in all patients with the maximum value varied from 468 to 8 000 U/L.Serum myositis antibodies were tested in 7 patients and all were negative.Electromyogram was performed in six patients with myogenic damage found in 2 patients.MRI of bilateral thigh muscle was carried out in six patients with muscle edema and mild fatty infiltration found in 2 patients.All patients underwent skeletal muscle biopsy with histochemical and immunohistochemical staining.The main muscular pathological features were muscle fiber atrophy,necrosis,regeneration and increased lipid droplets.Ragged blue fiber,cytochrome C oxidase-negative muscle fibers and decreased NADH activity were observed in some patients.MHC-Ⅰ expressed in the sarcolemma of muscle fibers at various levels.Mild C5b-9 staining was found in the endomysium,capillary and cytoplasm.Symptoms and the level of CK were improved in 7 patients after discontinuing statins or changing to another statin,while the immunosuppressive therapy were used in 2 patients and shown to be effective.Conclusions Statin induced myopathy is self-limiting in most patients,with improvement after discontinuation of statins.Few patients with autoimmune necrotic myopathy need immunosuppressive therapy.

ABSTRACT

ZnO nanotubes were prepared via electrospinning the Zn ( AC ) 2-polyacrylonitrile-polyvinylpyrrolidone ( PAN-PVP) precursor, followed by thermal decomposition of the above polymers from the precursor fibers. SEM and XRD characterization confirmed that the as-prepared ZnO nanofibers presented the hollow nanotube form, which was composed of ZnO nanoparticles with the size of about 40 nm in wurtzite crystal structure. By mixing with graphene, the obtained ZnO-graphene composite modified glassy carbon electrode ( ZnO-RGO/GCE ) was successfully constructed by dip-coating, which was used for the determination of Pb2+in water. With the sensitive response of the ZnO-RGO/GC electrode to Pb2+in solution was demonstrated by square wave stripping voltammetry, the response pctential was at -0. 4V. Under the optimized conditions, a good linear relationship between peak current and Pb2+ concentration was obtained in the range of 2. 4×10-9-4. 8×10-7 mol/L (R=0. 9970) by 10 min preconcentration at -1. 0 V in 0. 1 mol/L HAc-NaAc buffer solution (pH=4. 6). The detection limit was 4. 8×10-10 mol/L (S/N>3). The ZnO-RGO/GC electrode had good stability. The practical analytical application of the ZnO-RGO modified electrode was assessed by the measurement of the actual water sample and the result was consistent with that obtained by ICP-MS.

ABSTRACT

Objective To optimize the extraction process for the effective fraction of Bushen Yizhi ( BSYZ) formula. Methods Yield of the effective fraction and contents of two marker compounds 2,3,5,4 -tetrahydroxystilbene-2-O-β-D-glucoside and osthole assayed by high performance liquid chromatography (HPLC) were used as the optimizing indexes, and weighted coefficient of the indexes was analyzed by analytic hierarchy process ( AHP). Ratio of liquid-to-solid, extraction time, and extraction times were used to screen the optimal conditions by orthogonal test. Results The optimized extraction process of the effective fraction of BSYZ formula was as follows: extracting with 8-fold water for three times, and boiling for 1.5 hours each time. The verification test showed that the comprehensive scores of the optimized extractive process were higher than those obtained in the orthogonal test. Conclusion The optimized extraction process and conditions are practical in the manufacture of the effective fraction of BSYZ formula.

ABSTRACT

Objective:To establish an HPLC method to determine 5-amino salicylic acid (5-ASA) and its related substances in the enemas. Methods:The liquid chromatograph was equipped with a Hypersil ODS C18 column (250 mm × 4. 6 mm,5. 0 μm) with gradient elution. The mobile phase A was composed of pH 7. 5 phosphate buffer ( PBS), tetrabutyl ammonium hydroxide solution (TBAH), methanol and water (600∶50∶50∶300), the mobile phase B was composed of pH 7. 5 PBS, TBAH, methanol and water (200∶50∶300∶450). The wavelength was 208nm, the column temperature was 40 ℃ and the injection volume was 20μl. Results:5-ASA and its two degradation products could be completely separated. The concentration of 5-ASA and the corresponding peak area had a good linear relationship within the range of 1-250μg·ml-1(r=0. 999 5). The average recovery was 99. 44%(RSD=0. 94%,n=9). Conclusion:The method is simple, sensitive and reproducible, and can be used in the determination of the content and related substances in the enemas.
